All Players, Bench Staff and Parents attending facilities are required to complete a 48 HOUR RSVP and DAILY COVID-19 ATTESTATION AND AGREEMENT. The onus is on the parent and/or participant to ensure that they have completed the necessary forms.
The 48 RSVP must be competed no later than 48 hours prior to the event/practice/game)
The Daily Attestation must be completed no earlier than 8 hours prior to the event/ practice game and can be completed right up to the time of the event.
We will make every reasonable effort to make these Forms easy to access on many platforms.
